New Patients
Upper Valley Professional Corporation (UVPC) is the largest group of physicians in Miami County. More than 33 physicians practice at 14 locations. We provide high quality, cost-effective care to patients throughout Miami and Darke Counties.
Many UVPC physicians are currently accepting new patients. If you are considering a UVPC physician, visit the Find a UVPC Provider or Find a UVPC Practice page. There you will find information about the physician(s) in the practice, office hours and contact information so you can schedule your appointment.
Your First Appointment
New patients will be asked to complete a Patient Registration Form and a Personal Health History Form. These forms are available to download online if you would like to fill it out in advance. Use the Find a UVPC Provider directory to access your physician’s website and download the forms.
In addition, you may be asked to complete a Medical Records Release Form, which allows your new physician’s office to request medical records from your previous doctor. You may download the Medical Records Release Form. Complete the form and drop it off or mail it to your former doctor with a request that all your medical records be transferred to your new UVPC physician.

What to Bring With You
New patients should bring the following to their first appointment:
- Medications or a list of the medications you are currently taking
- A list of known allergies
- Photo identification
- Health insurance card(s)
- Patient registration forms (if you’ve filled them out in advance)
All patients should be prepared to pay for co-pays, deductibles and co-insurances at the time of service.
