Upper Valley Orthopaedics is accepting new patients and treats all of their patients with a friendly and courteous attitude. Our office is handicapped accessible.
To schedule an appointment with Upper Valley Orthopaedics, please call (937) 335-3561. Appointments with Upper Valley Orthopaedics may be made by the patient or referring physician.

- 8:00 am to 4:30 pm Monday through Thursday
- 8:00 am to 3:00 pm Fridays
Appointment/Procedure Preparation
Appointments with Upper Valley Orthopaedics physicians may be scheduled by the patient or referring physicians.
Please bring your insurance card, photo id, current medication list and any specialist copay.
What to Expect
When you arrive to our office at Upper Valley Orthopaedics, you will be greeted by friendly staff willing to help you with any questions you may have. Our staff will try to make your office visit as pleasant as possible.
New patients will be asked to complete a Patient Information form, a Past Family Social History form and a Review of Systems form.
During your visit to Upper Valley Orthopaedics, you will be examined by our physician to determine if additional treatment is needed. After your appointment you will check out with our receptionist to take care of any specialist co-pay. Any necessary follow up appointments will be made at this time.
After Your Visit/Procedure
If the physician recommends testing, surgery, physical therapy or occupational therapy, we will take care of this after your appointment. Our scheduler will contact you with details. If you have had surgery, you will be instructed on the day of surgery to call our office and schedule a follow-up appointment.
Billing and Insurance
Our office accepts most insurance and also handles workers compensation injuries. We will send the claim to your insurance company.
Between our staff and our billing department, we are happy to answer any questions you may have about your bill.
